This article discusses the complexities of antibiotic therapy against polyresistant bacteria, one of the most pressing problems of modern pharmacology and clinical medicine. The causes of the emergence of antibiotic-resistant microorganisms, the mechanisms of their spread, the difficulties in pharmacological treatment, and the need to create a new generation of antibacterial drugs are analyzed. The consequences of antibiotic misuse, the increase in hospital-acquired infections, pharmacokinetic and pharmacodynamic problems, and future prospects for antibiotic therapy are also widely covered. The role of pharmacovigilance, antibiotic stewardship programs, molecular diagnostics, and artificial intelligence technologies in eliminating problems associated with polyresistant bacteria is considered separately.
