Abstract

Ischemic heart disease (IHD) combined with obesity accelerates adverse myocardial remodeling through mechanisms involving visceral adiposity, systemic inflammation, and endothelial dysfunction. Epicardial adipose tissue (EAT) acts as a metabolically active visceral depot with local paracrine effects on the myocardium. Genetic polymorphisms in IL6, ZNF648, and ADIPOQ may influence inflammatory signaling, endothelial integrity, and adipokine-mediated cardioprotection, contributing to subclinical myocardial dysfunction.
